(Affidavit must be on 1000 Non Judicial stamp.) on format atteched Annexure -II Bank Defaulter : Submit an Affidavit on Rs 1000 Non judicial stamp before the1st class Magistrate confirming that Bidder is not defaulter from any Bank. Submit an affidavit before the 1 st class Magistrate/ Notary on stamp paper of Rs. 1000 (Non-judicial) stating that "In case any ambiguity is noticed in the documents submitted that any stage, we shall be entirely responsible and liable for any action as deemed fit under the law" shall be submitted by the Firm/Agency with Technical ? Notarize copy of Valid Electrical Contractor License and supervisor license of the suitable class in the name of the bidder or its associate issued from the competent government licensing authority to execute the electrical work. In case of bidder?s associate holding the license, it must state on Rs.1000.00 Non-judicial stamp from 1st class magistrate that all the electrical work must be done under its associate supervision with the signature and stamp of its associate.
