GENERAL PARTICULARS
Description Copy of GST Registration Copy of PAN Card Copy of TAN No. Copy of Labour License Affidavit/Self declaration of investment of 25% of Estimated Cost as per Tender Document. Value of existing commitments and on-going works to be completed during the period for which bid invited(Attach Supporting documents like work Order, Agreement Copy etc.) Affidavit regarding any litigation, debarred or blacklisted or history of rescinding work, no near relatives in the concerned division and the informations furnished in the bidding document is correct must be uploaded Income Tax Return of last 3 years FY 2020-21, 2021-22 & 2022-23 Annual Financial Turn over of last 05 (Five) Years [audited by Chartered Accountant] & shall be uploaded as per tender document. Experience of work of similar nature (Certified by concerned competent authority ) as per Section 2 of Tender Document. Timely completion certificate of completed work. The scanned copy of the Applicable terms of APPENDIX to ITB dully filled up & signed with seal must be uploaded The scanned copies of all the Applicable terms given in prequalifying terms in section 2 of the tender document (Qualification Information) (dully filled up & signed with seal ) must be uploaded . It must be mentioned. (Self Attested) (a) Details of tools & Plants :- (i) Concrete Nixture (ii) Vibrator (iii) Scaffolding (iv) Steel cutting and binding machine (plz attached as per format of clause 8.0 of section 2 of tender document ) (b) Quality control equipments:- (i) Compressive testing machine (ii) Compression testing machine and (iii) Slump test equipment Character Certificate(Issued from DM/SP) Evidence of access to financial resources must be uploaded(min 10% of estimated cost, certified by bankers, not more than 3 months old) as per 4.2(i) of ITB must be uploaded Proposed methodology and programme of construction as per 4.3(l) of ITB shall be uploaded Tender Document in full is accepted or not? If No, attach deviation letter stating only the points deviating from the Tender Document. If tender document in full is not acceptable and deviation letter is not uploaded than the tender may not be accepted. Bid shall remain valid for a period not less than 90 days after the deadline date. Bid validity for shorter period shall not be accepted Bank Solvency certified by banker as per 4.1 of Qualification Information (not more than 3 months old) List of key personnel ( In accordance with Annexure - II of tender document) Detail of work performed in last five years ( as per point 5 & 6 of section 2 of tender document) list of work pending work in COMFED/union/unit exceeding five (05) months beyond the ordered time schedule i.e work is delayed beyond five months of the time line specified in purchase order. If no work is pending then attach self declaration on letter head.
